Nicotinic acid binds to GPR109A (nicotinic acid receptor) on Langerhans cells in the skin, triggering prostaglandin D2 and E2 release → capillary vasodilation → redness, warmth and tingling. Nicotinamide has an amide group (–CONH₂) where nicotinic acid has a carboxylic acid group (–COOH).
